**Role Overview**
Our client — a cardiac care management MSO — is seeking experienced Registered Nurses to support a high-acuity population of patients with complex cardiac conditions, primarily congestive heart failure (CHF). The RN Care Manager is the clinical backbone of the care team, responsible for advanced clinical support through triage, care plan development, and high-risk care management across programs including Transitional Care Management (TCM), Principal Care Management (PCM), Complex Care Management (CCM), and Remote Patient Monitoring (RPM). This role also serves as the clinical escalation point for non-RN Care Coordinators, ensuring continuity and quality across the team.
**Key Responsibilities**
- Perform comprehensive clinical assessments via telephone for cardiac patients
- Develop and update individualized, evidence-based care plans
- Conduct clinical triage and determine escalation needs for worsening symptoms
- Lead Transitional Care Management (TCM) efforts post-discharge, including medication reconciliation
- Serve as clinical escalation point for non-RN Care Coordinators
- Provide patient and caregiver education on CHF self-management
- Assess home safety and social determinants of health (SDOH) barriers and escalate resource needs
- Coordinate with cardiologists, PCP offices, and community resources
- Document all patient encounters per CMS requirements using care management platforms
- Review population health dashboards to address care gaps (wellness visits, labs, symptom monitoring)
- Assist with advanced care planning, palliative care guidance, and hospice coordination as needed
**Experience ****&**** Skills**
**Required:**
- Active RN license in Texas or a compact state
- 2+ years of cardiac care or care management experience
- Strong knowledge of chronic conditions — especially heart failure and associated comorbidities
- Proficiency with EHR platforms and CMS documentation standards
- Familiarity with TCM, PCM, CCM, and/or RPM program requirements
- AI fluency — actively uses AI tools to work faster and more efficiently; specific tools and use cases confirmed at screening
- Time zone compatibility — must be available during US business hours (CST/PST overlap)
- Exceptional written and verbal communication in English; assessed at screening
**Preferred:**
- Based in or near the Dallas/Fort Worth area
- Experience with Remote Patient Monitoring (RPM) platforms
- Bilingual — Spanish/English (NOT A MUST)
